It affects your guarantees for own damage, theft, fire, vehicle glass and accidents not involving third parties with injuries or other damage.
- Your registration number or policy.
- Date, time and place of the incident.
- A brief description of what happened.
- Say whether any authorities were involved.
- Say whether the vehicle can be driven or is in the garage.
With a third party, with an accident report
If you have been involved in an accident with one or more other parties:
- Try to fill in the Accident report. The accident report form speeds up procedures between insurance companies and minimises the likelihood of disputes or discrepancies in determining liability for an accident.
With a third party, without an accident report
If you have not been able to fill in the accident report, you can use the services of the police or other authorities. Make a note of the minimum details of the other vehicle:
- Registration number.
- Make and model of the vehicle.
- If possible: Full name of the policyholder and/or driver, insurance company and policy no.

The accident report form speeds up the procedures between insurance companies and minimises the likelihood of disputes or discrepancies in determining liability after an accident.
The instructions for filling in the mutual declaration can be found in the mutual declaration itself, which you can download here. These are the most important ones:
- If both parties agree with the information in the declaration, you must sign it and make sure that the other driver does too. If you disagree with the information described or if it does not clearly and objectively reflect the facts, do not sign the declaration.
- Write down all the details of the persons concerned and witnesses (name, ID number, address, telephone number, etc.) on the back of the document. Since it is not signed, you can fill it in at a later date.
- Inform all those involved, whether the police or other authorities were involved or if there are more than 2 cars involved or if there are injuries to any person.
- Do not alter the declaration once the two copies (original and copy) have been separated. If you need to, you can contact us.
- Give a copy of the declaration to the other driver. Taking a photograph of the document is sufficient. In the event of more than 2 parties involved, you must have as many photographs or copies as the number of parties involved.
- Take photos of the damage and attach them when you report the claim to the insurance company.
- Contact us as soon as possible, and bear in mind that you have 7 working days to do so.
The declaration must not be amended in any way. Any change to the terms of the same must be notified to the company, Customer Service or to your agent.
It is very important to indicate that people have suffered injuries, even if they are not serious, in the accident report.
If anyone is seriously injured, call emergency services or the police.
If the emergency services (police, fire brigade, etc.) are involved, you must indicate this in the accident report.
It will be important to know which emergency service was involved in the accident and the number of the official report. Details of the officers are not necessary, since this information can be found using the report number, if necessary.
If there are several vehicles involved in the accident, please try to include the details of all the vehicles involved (number plate, driver, insurer).
Sometimes several accident declarations are made, depending on the order of the collisions. In this case, it is very important to provide documentation of all the declarations made (using photographs, copies, etc.).
